Forms

An Emergency Information and Permission Form is required for each student enrolled in an in-person class. This form may be submitted through any of the following methods:

• Complete the form online.
• Download the paper form to send via email/fax/mail.
• You may bring the form with you on the first day of class.

If you child requires an Epi-Pen or inhaler, please also complete the appropriate section of the Epipen and Inhaler Authorization Form. Please send it to our office via email/fax/mail, or bring it with you on the first day of class.
