The forms below are only required for students attending the Summer Program in person.
No forms are needed for students attending only the Online location.
An Emergency Information and Permission Form is required for each student enrolled in an in-person class. This form may be submitted through any of the following methods:
For your first session of the summer, our team will present you with a copy of the Summer 2022 COVID-19 Safety Procedures and Summer 2022 Community Agreement & Consent Form when you check in for your first session. Please review the digital version ahead of time by clicking on the links. If you wish, you may print this form out, fill it out, and bring it with you on the first day.
Since some of the symptoms associated with COVID-19 can be ambiguous - for example, when a child has seasonal allergies - please make sure to share this information on your Emergency Information and Permission Form. For forms that have already been submitted, please contact us with the changes you’d like to make.
If you child requires an Epi-Pen or inhaler, please also complete the appropriate section of the Epipen and Inhaler Authorization Form. Please send it to our office via email/fax/mail, or bring it with you on the first day of class.
